Put the white part inside the cover, close, sew or glue.
For an open book, just sew or glue the white part to the cover, then embroider the lines with black thread and the binding the same color as the cover.
Knot
MR, 2 ch, 6 trebles, 3 ch, 1 sl st (1st loop), then 2 ch, 6 trebled, 3 ch, 1 sl st (2nd looo).
Tighten the magic ring. Then wrap the thread around the centre of the MR several times. Sew/glue the knot to Marceline’s head.
